Universal Notice of Appointment of Advocate

To Whom it may concern: 

    1. This is a notice of appointment of an advocate for _______________________.

    2. ____________sought advocacy services from our church.

    3. An non-lawyer advocate was sought and approved from us to assist her/him advocating for himself/herself.

    4. The duration of this appointment is until she/he ceases to be willing to participate or cooperate with Universal, upon revocation by _______________, or until 5 years from the date of appointment.

    5. The relationship between _____________________ is confidential in nature and is of a member-church-pastoral nature. 

    6. The purpose of this appointment if to help him/her advocate for herself/himself and to advocate where needed for him/her.

    7. Advocacy is a broad term and herein expressly stated to help him understand community based resources and services and assist him/her with accessing those services in their complexity and securing services to thrive in the community.  This does not cover the full nature of advocacy services but provides agencies, organizations, and individuals the reason why our church help was sought and approved. Accessing services includes accessing clinical and medical services as needed.

    8. This is a member driven services and we can only help to the extent _____________ consents to assistance and expresses a desire for us to help in specified areas.
